Referrals and eligibility for professionals

Guidance for healthcare professionals on referral criteria, eligibility and how to refer patients to The Prince of Wales Hospice.

Supporting professionals to access the right care

The Prince of Wales Hospice works collaboratively with health and social care professionals to provide seamless specialist palliative and end-of-life care. Whether you’re referring patients for incare,  lymphoedema treatment or wellbeing services, this page gives you the information you need about eligibility and how the referral process works.

Services available through our hospice care

We provide a range of care and support services, from the first diagnosis until the end of life. Our services include:

  • Incare ward: Our Incare has 14 single bedrooms for 24-hour specialist nursing, medical and supportive care.

  • Multi-disciplinary team: A wide range of specialists who can provide expert advice and a holistic approach to people using our services.

  • Community wellbeing activities: Supporting patients and carers with social, activity-based and therapeutic groups

  • Lymphoedema service: Specialist management and treatment of lymphoedema

  • Counselling and bereavement support: Emotional support for patients, families and carers

These services are designed to support quality of life, symptom control and wellbeing in a holistic way.

Who can be referred

Incare

Eligibility for referral to our specialist palliative care services is based on patient need, not diagnosis. People may be eligible if they:

  • are adults (18+) with a life-limiting illness
  • have active, progressive and usually advanced disease for which the prognosis is limited and the focus of care is quality of life
  • have malignant or non-malignant disease, including patients with cancer, chronic lung or heart disease or a neurological disease, for example motor neurone disease
  • have unresolved complex needs that cannot be met by their current health care professionals. These may be physical, psychological, social and emotional
  • live in the catchment area for patients is the Five Towns area of Wakefield District. This includes Pontefract, Knottingley, Castleford, Featherstone and Normanton, plus Hemsworth, South Elmsall and South Kirkby. But we also consider referrals from out of region

 

Lymphoedema

People may be eligible if they live anywhere in the Wakefield District, and have a diagnosis of lymphoedema or chronic oedema.

Wellbeing support

People may be eligible if they:

  • are living with a life-limiting or progressive illness, or those who are important to the person experiencing these life-changing events

  • would benefit from social, emotional or therapeutic group support

  • Aae able to attend sessions in the community or hospice setting

  • may be experiencing isolation

  • may have frailty, reduced confidence or changes in health

 

Counselling and bereavement

People may be eligible if they:

  • are experiencing emotional distress related to illness or caring responsibilities

  • are coping with grief, anticipatory loss or bereavement

  • would benefit from one-to-one counselling or listening support

Who can refer

Incare

  • GPs
  • Hospital doctors
  • Macmillian nurses
  • District nurses
  • Specialist nurse

Ideally, an assessment will have been made by the hospital palliative care team or the Macmillan Community Team.

For other services, people can also self-refer.

How to refer to Incare and Lymphoedema

Referrals should be made on the district-wide referral form and emailed to referrals.pwh@nhs.net. Or by ringing 01977 708868. If you think it would be helpful for a patient or a carer to visit the Hospice before referral, we would be happy to show them around our facilities. Please call us on  01977 708868 to speak to our Nurse Manager and arrange a suitable time.

How to refer to any Wellbeing Services

You can refer someone, or people can self-refer. Please click on the button below and fill in the booking form. Alternatively, you can call the Wellbeing Team on 01977 781451.

After a referral is received

Once a referral is received:

  • Incare referrals are considered each weekday morning.

  • Urgent referrals will be considered on an individual basis.

  • Each referral is based on a number of factors, including patient needs and availability of beds.
  • We may contact you for clarification if needed.
