Referrals

Who can refer?

Referrals can be made by:

  • GPs

  • Hospital doctors

  • Macmillian nurses 

  • District nurses 

  • Specialist nurse

Ideally an assessment will have been made by the hospital palliative care team or the Macmillan Community Team.

 

How to refer

Referrals to Hospice services (excluding Lymphoedema) should be made by ringing 01977 708868  or can be made on the district-wide referral form, and emailed to referrals.pwh@nhs.net.


If you think it would be helpful for a patient or carer to visit the Hospice prior to referral, we would be happy to show them around our facilities, please call us on  01977 708868 to speak to our Nurse Manager and arrange a suitable time.


Lymphoedema referrals​

Referrals to our Lymphoedema service should be made through our Lymphoedema referral form, and emailed to referrals.pwh@nhs.net
 

Bereavement referrals

The Prince of Wales Hospice provides the Wakefield District Bereavement Support Service for the area. This service is available to anyone over 18, have been bereaved longer than four months and registered to a GP in the Wakefield area. Please, fill in the bereavement referral form or email to bereavement.pwh@nhs.net .
 

Admissions

Incare referrals are considered each weekday morning. Urgent referrals will be considered on an individual basis based on a number of factors including patient needs and availability of beds.
