Meet our Wellbeing Team

Wellbeing looks different for everyone. Our team takes the time to listen, understand your needs, and offer support that helps you feel more confident, connected, and in control.

Our Wellbeing Team is here to support people across the Five Towns area of the Wakefield District. We work together to listen, understand what matters to you, and offer practical help, emotional support, and support with physical wellbeing

Our Wellbeing activities

We are here to support people over 18 and their families across the Five Towns area of the Wakefield District, offering emotional, practical and therapeutic care for people living with life‑limiting illnesses, frailty and isolation or bereavement.

Wellbeing With Patient

We have a diverse team with different skills and experience to support you

You may meet one or several members of the Wellbeing Team, depending on your needs, preferences, and what support will be most helpful for you. Having the right support can make a real difference. Our joined‑up approach brings together emotional, physical and practical care to help you feel supported, understood, and less alone.
