How to get referred to The Prince of Wales Hospice

Clear guidance on hospice referrals for professionals, patients, and carers.

Who can make a referral?

  • GPs
  • Hospital doctors
  • Macmillan nurses
  • District nurses
  • Specialist nurse

Ideally, an assessment will have been made by the hospital palliative care team or the Macmillan Community Team.

If you are a patient or their family member, and you feel,  you need to be referred, please speak to your health care professional.

Who can be referred to the Hospice?

  • Adults (18+)
  • Have a life-limiting or progressive illness

  • Would benefit from specialist support alongside their existing medical care

  • Live within the Five Towns (including Pontefract, Castleford, Featherstone, Normanton, Knottingley and South Elmsall) and the surrounding area of Wakefield district

Support may be provided in a variety of ways, including inpatient care, community support, wellbeing services and family support.

How to make a referral

Incare referrals

Referrals to Incare can be made by ringing our Reception 01977 708868  or can be made on the district-wide referral form, and emailed to referrals.pwh@nhs.net.

Lymphoedema referrals​

Referrals to our Lymphoedema service should be made through our Lymphoedema referral form, and emailed to referrals.pwh@nhs.net.

Counselling and Bereavement Service referrals

Referrals can be made by calling 01977 781451 or by filling in the online referral form here. Please note, that the Bereavement sessions are offered only to people with a connection to our Hospice.
