Getting referred

Hospice services can be accessed via a referral from one of the following health care professionals: 

• GPs

• Hospital doctors (after assessment by the hospital palliative care team)

• Macmillian nurses 

• District nurses 

If you would like to be referred or would like to refer someone else, please talk to your GP or any of the above health professionals.
