How to refer to us?

Referrals to all services must be made on a referral form (click on pdf icon below). 

The hospice will accept referrals from District Nurses, Community Matrons, Specialist Nurses (both in the community and hospital), GP’s, Doctors, Consultants and allied health professionals.

Referrals are considered at a multi-disciplinary team meeting held each weekday morning by a combination of Doctors and in patient team.

Urgent referrals will be considered on an individual basis based on a variety of factors including patient needs and availability of beds.

It would be beneficial for the patient/carer to visit the hospice .To arrange a visit please contact the hospice
